== Therapeutic applications == Given the ability to knock down, in essence, any gene of interest, RNAi via siRNAs presents opportunities in both basic and applied biology. One of the biggest challenges to siRNA and RNAi-based therapeutics is intracellular delivery. siRNA also has weak stability and pharmacokinetic behavior. Delivery of siRNA via nanoparticles has shown promise. siRNA oligos in vivo are vulnerable to degradation by plasma and tissue endonucleases and exonucleases and have shown only mild effectiveness in localized delivery sites, such as the human eye. Delivering pure DNA to target organisms is challenging because its large size and structure prevent it from diffusing readily across membranes. siRNA oligos circumvent this problem due to their small size of 21–23 nucleotides. This allows delivery via nano-scale delivery vehicles called nanovectors. A good nanovector for siRNA delivery should protect siRNA from degradation, enrich siRNA in the target organ, and facilitate the cellular uptake of siRNA. The three main groups of siRNA nanovectors are: lipid based, non-lipid organic-based, and inorganic. LGD-4033, also known by the developmental code name VK5211 and by the black-market name Ligandrol, is a selective androgen receptor modulator (SARM) which is under development for the treatment of muscle atrophy in people with hip fracture. It was also under development for the treatment of cachexia, hypogonadism, and osteoporosis, but development for these indications was discontinued. LGD-4033 has been reported to dose-dependently improve lean body mass and muscle strength in preliminary clinical trials, but is still being developed and has not been approved for medical use. Known possible side effects of LGD-4033 include headache, dry mouth, adverse lipid changes like decreased high-density lipoprotein (HDL) cholesterol levels, changes in sex hormone concentrations like decreased testosterone levels, elevated liver enzymes, and liver toxicity. The potential of LGD-4033 and other SARMs for producing masculinization is largely uncharacterized and hence is unknown. LGD-4033 is a nonsteroidal SARM, acting as an agonist of the androgen receptor (AR), the biological target of androgens and anabolic steroids like testosterone and dihydrotestosterone (DHT). == Side effects and contraindications == The withdrawal of troglitazone has led to concerns of the other thiazolidinediones also increasing the incidence of hepatitis and potential liver failure, an approximately 1 in 20,000 individual occurrence with troglitazone. Because of this, the FDA recommends two to three month checks of liver enzymes for the first year of thiazolidinedione therapy to check for this rare but potentially catastrophic complication. To date, 2008, the newer thiazolidinediones, rosiglitazone and pioglitazone have been free of this problem. The main side effect of all thiazolidinediones is water retention, leading to edema, generally a problem in less than 5% of individuals, but a big problem for some and potentially, with significant water retention, leading to a decompensation of potentially previously unrecognized heart failure. Therefore, thiazolidinediones should be prescribed with both caution and patient warnings about the potential for water retention/weight gain, especially in patients with decreased ventricular function (NYHA grade III or IV heart failure). Though older studies suggested there may be an increased risk of coronary heart disease and heart attacks with rosiglitazone, pioglitazone treatment, in contrast, has shown significant protection from both micro- and macro-vascular cardiovascular events and plaque progression. ==== Absorption ==== The absorption of naltrexone with oral administration is rapid and nearly complete (96%). The bioavailability of naltrexone with oral administration is 5 to 60% due to extensive first-pass metabolism. Peak concentrations of naltrexone are 19 to 44 μg/L after a single 100 mg oral dose and time to peak concentrations of naltrexone and 6β-naltrexol (metabolite) is within 1 hour. Linear increases in circulating naltrexone and 6β-naltrexol concentrations occur over an oral dose range of 50 to 200 mg. Naltrexone does not appear to be accumulated with repeated once-daily oral administration and there is no change in time to peak concentrations with repeated administration.
The pharmacology of bicalutamide is the study of the pharmacodynamic and pharmacokinetic properties of the nonsteroidal antiandrogen (NSAA) bicalutamide. In terms of pharmacodynamics, bicalutamide acts as a selective antagonist of the androgen receptor (AR), the biological target of androgens like testosterone and dihydrotestosterone (DHT). It has no capacity to activate the AR. It does not decrease androgen levels and has no other important hormonal activity. The medication has progonadotropic effects due to its AR antagonist activity and can increase androgen, estrogen, and neurosteroid production and levels. This results in a variety of differences of bicalutamide monotherapy compared to surgical and medical castration, such as indirect estrogenic effects and associated benefits like preservation of sexual function and drawbacks like gynecomastia. Bicalutamide can paradoxically stimulate late-stage prostate cancer due to accumulated mutations in the cancer. When used as a monotherapy, bicalutamide can induce breast development in males due to its estrogenic effects. Unlike other kinds of antiandrogens, it may have less adverse effect on the testes and fertility. In terms of pharmacokinetics, bicalutamide is well-absorbed when taken by mouth. In his widely awaited first speech since the start of the war in Gaza on 3 November, Hezbollah leader Hassan Nasrallah said the presence of U.S. warships in the Mediterranean "doesn't scare us". On 5 November, Hezbollah shot down an Israeli Elbit Hermes 450 drone over Nabatieh, with wreckage falling over houses in the towns of Zabdin and Harouf. One Israeli civilian was killed when anti-tank missiles hit the Yiftah kibbutz. Four people were reportedly injured after an Israeli bombing that hit two ambulances. Later, an Israeli airstrike hit two civilian cars in Lebanon between the towns of Aynata and Aitaroun, killing one woman, three of her granddaughters between the ages of 10 and 14, and severely injured her daughter. In response, Hezbollah fired at Kiryat Shmona, killing an Israeli civilian. The Al-Qassam Brigades took responsibility for firing 16 rockets from Lebanon targeting areas south of Haifa. Meanwhile, Israel reported at least 30 rockets being fired in which the IDF fired back at the sources. Hezbollah and Al-Qassam brigades also conducted four cross-border attacks into northern Israel. Hezbollah deputy general Naim Qassem said that the group could be forced into wider conflict over Israeli attacks in Gaza. On 10 November, Hezbollah launched anti-tank missiles into an IDF post in Manara which injured three soldiers. The IDF attacked the sources of fire in response. Hezbollah conducted three drone attacks into northern Israel targeting IDF positions and civilians. One drone was intercepted while two others landed on Israeli territory.
